Consent forms

Get consent signed, and keep it.

The patient signs on a tablet at the chair or on their own phone. The signed form stays on their file, so there is no paper to file or lose.

Sign at the chair, or on the patient's phone

Hand over a tablet to sign, or send a signing link the patient opens on their own phone. The form reads in Arabic or English, whichever you prepared it in.

I have read this form, I understand it, and I agree to it.

  • Sign on the screen at the chair
  • A signing link by WhatsApp, email or a QR code, valid for a set time
  • A guardian can sign for a child, with their name and relationship
  • Written in the language you prepared the form in

Consent you can stand behind later

The wording is fixed the moment the form is prepared, and the signed record carries a fingerprint. If anything changes afterwards, the record says so.

  • The exact wording the patient saw is kept, even if you later edit the template
  • A signed form is never edited or deleted, only voided with a reason
  • The dental chart records the mouth as it was before the treatment
  • Every form shows whether it is awaiting signature, signed, declined or voided

Require it before the procedure

Set up the wording you use once, and say which procedures each form is required before. You start from a ready set of templates.

  • A starter set of templates, in Arabic and English, that you can change
  • Mark a template as required before certain procedures
  • Choose whether an unsigned form blocks the treatment or only warns
  • A form can cover a whole treatment plan, which the patient accepts by signing
